Payment Financing

We are committed to providing excellent dental care and the finest services possible for all of our patients.

Payment is required at the time services are performed. If you will be receiving a new denture, payment must be made after the impression for your denture is taken.

This system greatly reduces the billing and collection costs and results in fees that are lower than would otherwise be necessary. Please do not hesitate to ask us in advance about fees for planned services.

For our patients with or without insurance

Option 1

Cash or check at the time services are rendered.For patients with insurance, as a courtesy we will file your insurance claim for your reimbursement.

Option2

Discover Card, Master Card, Visa.

If you desire we will assign insurance payment for us from most insurance plans.

Predetermination is mandatory before we can accept insurance payment. Insurance status must be revealed on first visit.

A $25 NSF fee applies to all bank returned checks.

Appointment Policy

The nature of our practice is to provide extremely high quality care that require at times, longer appointments for each of our patients visits.

It is our policy that 24 hours notice must be given if you are forced to cancel an appointment. Longer crown and bridge appointments, surgery,and scaling and root planings require a 48 hour notice.

After 2 broken appointments with no notice, we will place your file in an "inactive status" and special arrangements must be made to reactivate it.

OFFERS MAY BE COMBINED. OFFERS HAVE NO CASH VALUE. THE PATIENT AND ANY OTHER PERSON RESPONSIBLE FOR PAYMENT HAS A RIGHT TO REFUSE TO PAY, CANCEL PAYMENT, OR BE REIMBURSED FOR PAYMENT FOR ANY OTHER SERVICE, EXAMINATION, OR TREATMENT THAT IS PERFORMED AS A RESULT OF AND WITHIN 72 HOURS OF RESPONDING TO THE ADVERTISEMENT FOR THE FREE,DISCOUNTED FEE, OR REDUCED FEE SERVICE, EXAMINATION OR TREATMENT.
